- Address (Do NOT include your door number):
- Description of your pet:
- Relevant medical or care information for the pet (Diet/Allergies):
- Please attach a photo of your pet (This is important to easily identify your pet)
Purpose of Data Usage:
The information provided is used solely to help identify your pet and contact you in the event your pet is found. A QR code linking to a secure web page with these details will be generated and may be attached to your pet’s collar or ID tag.

By completing and submitting your pet details and your contact informations, you agree to allow WhiskerPatroller to publish the details you provide for the purpose described above. - Public Accessibility:
The QR code may be scanned by any person who finds your pet, leading to a page that displays the shared information. - Right to Withdraw:
You may request the modification or removal of your information at any time by contacting WhiskerPatroller WhatsApp. Deactivation of your QR profile may take up to 1-5 business days once your message has been seen. - Data Responsibility:
You are responsible for ensuring that the information you provide is accurate and up to date. Please send us a message anytime to update your details. - Security & Privacy:
We will not share your information for marketing purposes or with third parties beyond the scope of the pet recovery service.
